Get Started on Becoming an APD Provider

If you or someone you know is interested in becoming a provider for iBudget Waiver services, please visit the APD Quick Guide: Applying to Becoming an APD Provider. To apply to be an iBudget Waiver service provider, please take the following steps:

• Download the appropriate APD application by navigating through the APD Quick Guide here.
• Register and obtain all background screenings. For background screening information, please see our background screening webpage here.
• Fill out provider application thoroughly and legibly.
• Gather all supporting documentation, utilizing the checklist contained inside the application. Additional forms that will be needed are below:
• Provider Enrollment Personal Reference Form
• APD Attestation of Good Moral Character
• Submit the application and supporting documentation to the Provider Enrollment email address below for the region where you are applying to render services:

Provider Expansion Requests for Current APD Providers

In order to expand, Providers must meet the minimum qualifications per the iBudget Handbook.

APD Expansion Requests are processed in the APD iConnect system. Please click on the following manual pertaining to the type of Expansion you are requesting:
