After you select "Send Form" button, you will be redirected to the QSI Web Based Training closing page.

Once you are on this page, there are instructions for printing your Level 2 WSC Access Authorization Request Form.

You will need to print a copy of the Level 2 WSC Access Authorization Request Form AND of your Confirmation Email that will be sent to the email address you provided above.

You will be required to show both of these documents as proof of your registration.
