It provides us with an identification number that no one else will have. It provides us with verification that you are a citizen or a legal permanent resident of the United States. We will not ask for the Social Security number of anyone else in your household who is not requesting services from the Agency for Persons with Disabilities. What Will We Do With Your Social Security Number? Your Social Security number will be maintained in a safe place where others cannot see it. Your Social Security number will be your identification number in our client information system. Only APD employees and Waiver Support Coordinators use this system. Your Social Security number will appear on the authorizations that we give to your service providers so they may bill Medicaid for services provided to you. We will not use or give out your Social Security number for any reason, including referrals to other agencies that may be able to help you, unless you have given consent in writing. If you feel that your Social Security number has been used improperly, or if you have questions about the use of your Social Security number, please contact your Waiver Support Coordinator or local APD area office.
